Route 1: Referral
The social worker or authorized person will send the application form, summary of the applicant's case and psychological report by post or fax to the Centre.
Route 2: Individual Application
By post / fax / in person to submit the service application form and psychological report.
● Upon completion of the training, the center will arrange for referral and follow up for 6 months and then close the case;
● If parents request withdrawal, centre will discuss with parents and referral arrangement.

Applicants may apply for the service through referrals (e.g. Central Referral System for Rehabilitation Services of the Social Welfare Department) or by themselves to our social workers.
